Advanced visualization techniques could change the paradigm for diagnosis and treatment of heart disease

ScienceDaily (May 31, 2012) ? Researchers from Mount Sinai School of Medicine are pioneering new ultrasound techniques that provide the first characterization of multidirectional blood flow in the heart. By focusing on fluid dynamics -- specifically, the efficiency with which blood enters and exits the heart's left ventricle -- the researchers believe they can detect heart disease even when traditional measures show no sign of trouble.

In addition to improving diagnoses, this shift in focus from muscle mechanics to fluid mechanics could lead to more effective therapeutic interventions. The work is described in a study published by two Mount Sinai cardiologists and a team of international collaborators in a recent issue of JACC Cardiovascular Imaging, a journal of the American College of Cardiology.

The ultrasound tools cardiologists use today often fail to detect changes in the heart until there is overt dysfunction. Blood flow imaging, however, may provide better clues in diagnosing heart failure. Sinai investigators reason that flow should be immediately affected by changes in cardiac function -- such as those revealed in image analysis by the chaotic behavior of tiny whirlpools.

The computer-aided visual study of these abnormalities could dramatically improve the assessment of patients with heart failure and lead to a fresh understanding of normal and abnormal pumping and circulatory function. Visual blood-flow analysis could also yield improved therapies for arrhythmias and other disorders requiring cardiac synchronization. Researchers are actively exploring applications in aortic atherosclerosis, before and after valve replacement, and congenital abnormalities.

"With visualization, we are looking at the ultimate measure of the efficiency of the heart -- how the blood is brought in and how it is sent out," said Jagat Narula, MD, PhD, Director of Cardiovascular Imaging at Mount Sinai and the senior author of the paper. "Today, cardiologists place great weight on a gauge called the squeeze fraction, or ejection fraction -- the portion of blood pumped from the ventricle with each heartbeat. What we are doing is a complete departure from the view of the heart as a squeezing, pressure-generating chamber."

Mount Sinai researchers and their collaborators have experimented with a range of imaging techniques to grasp the characteristics of normal and abnormal blood flow. The approaches include phase-encoded MRI, cardiac magnetic resonance (CMR) and several forms of ultrasound-based imaging known as echocardiographic particle imaging velocimetry.

"The most effective technique involves injecting a stream of bubbles that behave exactly like red blood cells and using echocardiography to track their path through the left ventricle," said Partho Sengupta, MD, Director of Cardiac Ultrasound Research at Mount Sinai, and the first author, with Narula, of the JACC paper. In these investigations, the computer-enhanced video output depicts normal and turbulent flow in vivid detail, with arrows plotting the direction as the bubbles swirl through the heart chamber.

"Not only are you following the path of the blood, but you can actually identify the amount of energy that is being distributed," said Dr. Sengupta. "Like other forms of ultrasound, that means low-cost heart tests using this technology could be performed on a simple outpatient basis."

The echocardiography technology pioneered by Sengupta and Narula sheds light on diagnostic discrepancies that have puzzled cardiologists relying on pressure measurements.

"After sustaining significant damage, a patient's heart may not have the greatest squeeze, but there could be good trafficking of blood through the heart and the patient could remain asymptomatic," Sengupta explained. "The normal ejection fraction is around 60 percent, but we sometimes see a patient with 20 percent walking around and playing golf. Other people who are at 50 percent may be short of breath. Flow visualization is one way to capture the essence of why the patient is or is not symptomatic."

Diagnosing cardiac disease by looking for structural defects in the heart is like analyzing highway traffic by examining the road, Dr. Narula said. "The structure may not be great, but how does that affect the cars that are actually traveling on the road? It's the same thing if you fail to look at the blood."

Likewise, a plumber's investigation of pipes in a house only matters or makes sense in relation to how the water flows, claims Sengupta. A new study these investigators have submitted for publication zeros in on specific correlations between blood flow and cardiac pathology. "We will be able to demonstrate that efficiency may be lost even though the structure is maintained," said Senguptra. "In other words, the fa?ade is good, but inside, you have lost it."

Sengupta points out that the combined visualization and computation techniques in the JACC paper are still new and require further work, including development of appropriate flow-based indexes for applications in various cardiac pathologies. Forces acting on flow are exceedingly complex and dynamic, the researchers said. Pumped by the heart at a rate of 8 pints to 16 pints per minute, blood interacts with the contours of the myocardium, valves, vessels, and other features, which are also in motion. The flow is multidirectional -- curling, spinning, and forming eddies that are affected in countless ways by structural changes in heart tissue. As with any new observational techniques, data from novel cardiac visualizations in complex environments are subject to interpretation.

"We have started using these imaging techniques in clinical trials," Narula said. "They will require careful evaluation."

Mass. woman finds baby bird with 2 heads, 3 beaks

(AP) ? A Northampton woman made an unusual discovery in her yard ? a deformed baby cardinal with two heads and three beaks.

April Britt says she found the bird near the base of a tree Monday and returned it to its nest.

The 67-year-old Britt tells the Daily Hampshire Gazette (http://bit.ly/LFlSqn ) the bird apparently couldn't eat without assistance from the adult birds. The middle beak didn't work well and the other two didn't appear to be connected to the bird's throat.

A scientist for the Massachusetts Audubon Society says animals with such abnormalities rarely survive birth, making the find all the more unusual. He says the deformities could have been caused by genetic or environmental factors.

The baby and adults were gone by Tuesday.

Netflix for iOS updated with a new player UI, support for alternate language audio

There's still no word available on HD video streaming, but Netflix has just released version 2.2 of its app for iPhones and iPads. While the front end got tablet friendly in December and received a Retina Display makeover in March, this version brings a new player UI to all iOS devices. The new scrub bar seems much easier to reach and control now, while the button for subtitles has a received a tweak reminiscent of the new look recently pushed to Netflix's web based player. Also under that button is a new option allowing for alternate language soundtracks (where available), while the changelog goes on to mention "general performance improvements". Not documented but present is the addition of thumbnails in that scrub bar, easier access to "do not share" for Facebook-connected members and the ability to restrict streaming to WiFi-only in the settings. The new version is shown above, check after the break for a grab of the old player to see the difference. As always, the latest update can be found in the app store, we'll let you know if we hear of any extra details or changes on other platforms.

Scotland Real Estate Set To Drop With End Of Stamp Duty Exemption

The stamp duty exemption for Scotland real estate purchases is at an end, and it is expected that market will see a significant drop in activity now. 38% of first time buyers in Scotland purchased homes in the ?125,000 to ?250,000 range - which was the range exempted from stamp duty. First time buyers in Scotland bought higher priced property than usual in the first quarter of 2012, taking advantage of the stamp duty concession, according to new figures from the Council of Mortgage Lenders in Scotland.

Some 38% of first time buyers in Scotland bought property valued between ?125,000 and ?250,000, the bands exempt from stamp duty during the concession. This is the highest proportion since records began in 2005.

Due to lower house prices in Scotland, around 35% of first time buyers usually buy property in these bands compared to 50% in the UK as a whole. Properties under ?125,000 were still the most popular option in Scotland in the first quarter but, at 58% of first time buyers buying within this band, this is the lowest since records began.

There were 4,000 loans, worth ?380 million, taken out by first time buyers in Scotland in the first quarter. Despite the stimulus provided by the end of the stamp duty concession, this was a 9% fall in the number of loans and a 5% fall in the value compared to the previous quarter, but a rise of 21% by number and 31% by value compared to the first quarter of 2011.
First time buyers accounted for 42% of the house purchase loans in Scotland, the highest since 2001 and the same proportion in the UK as a whole.

Scottish first time buyers typically borrowed 80% of their property's value in the first quarter of the year, the same as the UK overall. The proportion of income spent on mortgage interest payments by first-time buyers increased to 11.4% in the first quarter from 11% in the previous quarter but was still lower than the proportion paid by first time buyers across the UK which was 12.7%.

Lending to home movers followed a similar pattern to first time buyers. The 5,500 loans taken out, worth ?720 million, was a fall of 24% (25% by value) from the last quarter, but a rise of 8% (11% by value) compared to the first quarter of 2011.

Overall, Scotland experienced a fall in lending for house purchase, as in the UK, in the first quarter of 2012. 9,500 loans were taken out, worth ?1.1 billion, down from 11,600, worth ?1.4 billion, in the last three months of 2011, but up from 8,400, worth ?930 million, in the three months to March 2011.

Remortgage lending also dropped in the first quarter in Scotland and UK wide. There were 7,800 loans, worth ?750 million, down from 9,000, worth ?880 million, the previous quarter and 8,900, worth ?850 million, in the first quarter of 2011.

?Despite the fall in lending in Scotland in the first quarter of the year, the end of the stamp duty concession clearly had an effect on the market, mainly with the increase in the proportion of higher priced properties bought by first time buyers during this period,? said Iain Malloch, chair of CML Scotland.

?However, if Scotland follows the trend expected across the UK, there is likely to be a further drop in activity over the coming months. The CML in Scotland looks forward to working with the Scottish government and other stakeholders throughout the rest of 2012 to ensure the housing market is open for business across all tenures,? he added.

PITTSBURGH (AP) ? Imagine a tiny snake robot crawling through your body, helping a surgeon identify diseases and perform operations.

It's not science fiction. Scientists and doctors are using the creeping metallic tools to perform surgery on hearts, prostate cancer, and other diseased organs. The snakebots carry tiny cameras, scissors and forceps, and even more advanced sensors are in the works. For now, they're powered by tethers that humans control. But experts say the day is coming when some robots will roam the body on their own.

"It won't be very long before we have robots that are nanobots, meaning they will actually be inside the body without tethers," said Dr. Michael Argenziano, the Chief of Adult Cardiac Surgery at New York-Presbyterian Hospital and Columbia University Medical Center in New York.

Argenziano was involved with some of the first U.S. Food and Drug Administration clinical trials on robotic heart surgery more than 10 years ago. Now he says snake robots have become a commonly used tool that gives surgeons a whole new perspective.

"It's like the ability to have little hands inside the patients, as if the surgeon had been shrunken, and was working on the heart valve," he said.

But Argenziano and experts in robotics say the new creations work best when they're designed for very specific tasks. "The robot is a tool. It is no different in that sense than a scalpel. It's really a master-slave device," he said.

Howie Choset has been researching and building robots, particularly snake robots, at Pittsburgh's Carnegie Mellon University for years.

Choset believes that his snake robot and others like it help reduce medical costs by making complex surgeries faster and easier. Choset says his new design is smaller and more flexible than earlier models: The diameter of the head is less than the size of a dime.

The size of surgical robots allows surgeons to operate with far less damage to the body, helping the patient heal faster. For example, instead of opening the entire chest up during heart surgery, a small incision is made, and the robot crawls inside to the proper spot.

Dr. Ashutosh Tewari of Cornell University Medical Center has used robotic tools to perform thousands of prostate operations. He said the precision of the tiny robotic tool is vital not just to cutting out cancerous tumors, but to seeing exactly what nerves to leave intact.

Tewari said he's most excited about the potential for surgical robots to do things humans can't do. He said the variety of sensors available for surgical robots keeps expanding, even as they get smaller. He said they may one day be able to test chemicals or blood in the body, or even the electrical connections in nerves.

Choset has also built larger snake robots designed for search and rescue, or just exploration. They can climb poles or trees and then look around through a camera in the head, and slither through places humans can't reach.

"We sent our snake robots into these caves off the coast of the Red Sea to look for evidence of ancient Egyptian ships," he said. "To me archaeology is like search and rescue, but everyone's been dead for 5,000 years," reducing the pressure.

Another expert at Carnegie Mellon stresses that there's still an enormous gap between humans and even the most high-tech robots. Manuela Velosa noted that robots have been built that excel at one or two tasks ? but not at the variety of tasks humans perform without even thinking.

Velosa has been building robots that ask humans for help when they don't know what to do, as well as teams of robots that play soccer against each other. Sometimes, the robots surprise her.

During one soccer game against robots from another university, the Carnegie Mellon team scored on a particular play. That sent a positive signal to the robot's computers, which are designed to reward success and discourage failures in the game.

Her robots then tried the play again ? and scored again. It turned out they had discovered a programing flaw in the other team of robots, just like some sports teams find a flaw in their opponents.

In the second half Velosa's robot's kept using the same play, scoring every time, and thus reinforcing the tendency to try the play over and over. The robots crushed the other team.

"It was programmed by me, but it looked to me as if they learned," she said. "I believe we are much closer to having robots be able to coexist with humans. The beautiful thing is you see the robots learning."

UN Security Council condemns Syria massacre

Syria blamed terrorists for the Sunday massacre of more than 100 people, including children. Washington isn't buying it. NBC's Richard Engel reports.

Updated at?6:01 p.m. ET -- The U.N. Security Council on Sunday unanimously condemned the Syrian government for heavy-weapons attacks on the town of Houla, the site of a massacre of at least 108 people, including many children, the council president said.

"The Security Council condemned in the strongest possible terms the killings, confirmed by United Nations observers, of dozens of men, women and children and the wounding of hundreds more in the village of (Houla), near Homs, in attacks that involved a series of government artillery and tank shellings on a residential neighborhood," the non-binding statement said.

"The Security Council also condemned the killing of civilians by shooting at close range and by severe physical abuse," said the statement, which was read out after the council's three-hour emergency meeting by Azerbaijan's Deputy U.N. Ambassador Tofig Musayev.

Facing mounting international outrage over the killings, Syria earlier on Sunday accused rebels of carrying out the massacre.

Images of bloodied and lifeless young bodies, lain carefully side by side after the onslaught on Friday, triggered shock around the world and underlined the failure of a six-week-old U.N. cease-fire plan to stop the violence.

Syrian authorities blamed "terrorists" for the massacre, among the worst carnage in the 14-month-old uprising against President Bashar al-Assad, which has cost about 10,000 lives.

"Women, children and old men were shot dead. This is not the hallmark of the heroic Syrian army," Foreign Ministry spokesman Jihad Makdesi told reporters in Damascus.

Makdesi said Syrian security forces were in their local bases Friday when they were attacked by "hundreds of heavily armed gunmen" firing mortars, heavy machine guns and anti-tank missiles, staring a nine-hour battle that killed three soldiers and wounded 16.

Opposition activists said Assad's forces shelled Houla after a protest and then clashed with fighters from the Sunni Muslim-led insurgency.

Activists say Assad's ?shabbiha' militia, loyal to an establishment dominated by members of the minority Alawite sect, then hacked dozens of the victims to death, or shot them.

Maysara al-Hilawi said he saw the bodies of six children and their parents in a ransacked house in the town.

"The Abdelrazzak family house was the first one I entered. The children's corpses were piled on top of each other, either with their throats cut or shot at close range," Hilawi, an opposition activist, said by telephone from the area.

"I helped collect more than 100 bodies in the last two days, mostly women and children. The last were six members of the al-Kurdi family. A father and his five kids. The mother is missing," he said.

Footage broadcast by activists in the Damascus suburb of Yalda showed a crowd of hundreds at one of the men's funerals shouting "the people want the downfall of the regime."

The U.N. Security Council held an emergency meeting Sunday to discuss the killings.

The United Nations believes that at least 108 people were killed in?the massacre, the U.N. peacekeeping chief Herve Ladsous told the meeting.

U.N. observers in Syria have confirmed that artillery and tank shells were fired, U.N.?Secretary-General Ban Ki-moon said in a letter to the Security Council.

The letter, which was obtained by Reuters, said the observers "viewed the bodies of the dead and confirmed from an examination of ordnance that artillery and tank shells were fired at a residential neighborhood."

Western countries and Arab states opposed to Assad put the blame for the deaths squarely on Damascus.

The Gulf Cooperation Council of Sunni-led monarchies accused Assad's soldiers of using excessive force and urged the international community to "assume its responsibilities to halt the daily bloodshed in Syria."

EU foreign policy chief Catherine Ashton spoke of a "heinous act perpetrated by the Syrian regime against its own civilian population" in a statement on Sunday. The head of the European parliament said it could amount to a war crime.

'Rule by murder'
"We are horrified by credible reports of targeting killing, including stabbing and ax attacks on women and children in Houla. These acts serve as a vile testament to an illegitimate regime that responds to peaceful political protest with unspeakable and inhuman brutality," U.S. National Security Council spokesman Tommy Vietor said Sunday.

U.S. Secretary of State Hillary Clinton demanded that those who carried out the killings be held to account.

"The United States will work with the international community to intensify our pressure on Assad and his cronies, whose rule by murder and fear must come to an end," she said.

France said it would call a meeting of the Friends of Syria, a group of Western and Arab countries keen to see Assad removed.

Britain said it would summon Syria's envoy over the massacre and that it would call for a meeting of the U.N. Security Council in coming days.

The United Arab Emirates requested an urgent meeting of the Arab League, whose head, Nabil Elaraby, urged the U.S. Security Council to stop the killing.

Former U.N. Secretary-General Annan is to brief the U.N. Security Council on Wednesday and is likely to be guided on who is responsible by reports from U.N. observers in Houla.

Although the cease-fire plan negotiated by Annan has failed to stop the violence, the United Nations is nearing full deployment of a 300-strong unarmed observer force meant to monitor a truce.

The plan calls for a truce, withdrawal of troops from cities and dialogue between government and opposition.

Syria calls the revolt a "terrorist" conspiracy run from abroad, a veiled reference to Sunni Muslim Gulf powers that want to see weapons provided to the insurgents.

The United Nations has accused Assad's forces and insurgents alike of grave human rights abuses, including summary executions and torture.

We all know that there is a growing need in this country to take our retirements into our own hands if we want the funds necessary to have any quality of life upon retirement. The problem is that most of us have no idea where to begin when it comes to financial retirement planning or investing. The sad news is that for most of our lives retirement was something that was taken care of if we put in an honest lifetime of work. However, the climate has changed and the retirement funds that many of us have labored to pay for the vast majority of our lives are slipping away.

The good news is that this need has not gone unnoticed by the powers that be and while they aren?t offering solutions for the funds we?ve already invested or in salvaging what is left of the failing system, they are empowering people to take some control for their personal retirements by offering investment options and strategies that provide tax benefits along the way in order to reward you for your efforts.

The four common types of retirement plans include 401(K) plans, Keough Plans, IRAs (individual retirement accounts), and qualifying pension or profit sharing plans offered by corporations. In most retirement plans, the contributions to those plans are tax deductible and taxes aren?t paid on these plans until the funds are received and retirement payment begins. You should be careful of your investments and guard them well as there are often hefty penalties involved when you take funds out of your retirement funds before you actually retire.

These of course are not the only types of investments you can make for your golden years and it never hurts to have more eggs in many baskets. The more the merrier in most cases. My personal preference for investing is real estate. This is an investment that you can actually see and reach out and touch. It is also an investment that often gets overlooked when planning for retirement, though when you consider it is an excellent choice. Property values are much lower today than they will be ten, twenty, or fifty years from now. This means the sooner you buy the property the more it will be worth (in theory) when you retire. The thing to remember is that property investing, like other types of investing, requires some degree of risk. You need to learn as much as you can about the process and discuss your interest with a financial advisor before you make any major decisions concerning your retirement investments.

There are more traditional investment methods you may want to consider as well. Mutual funds and the stock market are great ways to invest your money, build a decent portfolio, and increase your net worth. This type of investing also carries some degree of risk and isn?t always considered financial retirement planning but more along the lines of simple financial planning.

The thing to remember is that it is always good to have a plan. For this reason, I strongly encourage you to engage the services of a good financial planner. He or she can help you navigate the tricky language that is involved in many transactions, set realistic and obtainable retirement goals according to your needs as well as your means, and offer excellent advice and guidance on other investment ventures you may wish to pursue. In other words, a good financial planner can help you plan for your retirement.

When it comes to the world of finance, many of us are far from experts. We seek legal advice from attorneys, tax advice from accountants, and medical advice from doctors yet very few of us go to financial planners when planning our financial retirement. In many ways it makes little sense to approach our futures so carelessly and yet this is not something that our parents and grandparents would have done so there is no precedence for doing so. The problem is that money is such a limited commodity in this world, we are living longer than ever before, and we are enjoying much more mobility in our golden years than in times long past. Getting Out in the Community

BY GAVAN CONWAY ? During the recent visit by Kaipo Ho and Marsha Kapu, the Australian Corporate Office staff decided to undertake some community work for a day.?The office was divided into two groups each having the task of finding a community organisation where they could volunteer for a day.

Marsha joined the first group who spent the day with Friends of Federation Walk, a Gold Coast City initiative for the regeneration of the vegetation on The Spit, which is a beach and dune area near Surfers Paradise. Their objective is to maintain Federation Walk Coastal Reserve as a natural Public Open Space for the benefit of the community. It is a restoration and low impact conservation project that the community can be actively involved in. Their duties included planting over 50 new trees and cleaning up pathways along the foreshore.

The second group, which included Kaipo, went to the Animal Welfare League (AWL) where they helped to clean out pens and walk dogs. Of course, there were one or two puppies and kittens to play with! The AWL has a policy of not euthanizing stray animals and attempts to find new homes for all animals in their care. On the day we visited the shelter, it was temporary home to dogs, cats, rabbits, guinea pigs, chickens and an uninvited green tree snake (frightening but non-venomous).
