Survival after a diagnosis of AJCC Stage IV prostate cancer ? THE ...

For many men, a diagnosis of prostate cancer of any stage or grade ? even of the lowest level of risk ? is hard to deal with. If they have progressive disease after first-line therapy, it may appear to be an overwhelming issue.

This morning, on Ask Arthur, we received a pleasant surprise, which may offer great hope and a ?real life? perspective for the many men who do, indeed, find that first-line therapy has failed to eliminate their cancer, and that they have a rising PSA ?

We will let you read the actual tale for itself. However, for those of you who haven?t been dealing with prostate cancer since the mid to late 1990s, there was an earlier time ? and an earlier version of this web service. About the only thing in common between the two was ?Ask Arthur.? Then, as today, Arthur sought to help individual men with questions about their diagnosis and treatment. Today we also provide a much wider service too ? through the use of our social network, and the calls on Arthur?s time and expertise are far fewer than they were back in 1995 to early 1998, when the original InfoLink site was one of few places you could get to if you entered ?prostate cancer? into the relatively primitive search engines of that time.

The point to all this, however, is that progressive prostate cancer is far from being the end of life. The gentleman who left the message for Arthur last night was initially diagnosed in his mid to late 40s. By 1997 he had a rising PSA, had been told he had Stage IV disease, and that he had 2 to 3 years left to live. But not at all.

Prostate cancer is an unusual disease, and we still know little about the biological drivers of prostate cancer in specific individuals. What we do know, however, is that most men diagnosed with prostate cancer today are not going to die from this disease if it is appropriately managed; that many men diagnosed even with late-stage, progressive disease, may live for a decade or more after their diagnosis; and that we are at the beginnings of a revolution in the management of progressive and advanced forms of prostate cancer, as may be discovered through innovative trials like this one, that has just started to recruit men with relatively early stage metastatic, castration-resistant prostate cancer to bone only.

It is also worth noting part of Arthur?s response to his correspondent:

Arthur believes that the cultural and social biases that kick in (among patients themselves, among family members, and among the professional community) when any diagnosis of cancer is given often still reflect the mindsets of the 1950s as opposed to the 21st Century.

?Cancer? is a loaded word. We do indeed often respond to it in ways that demonstrate our misconceptions about the implications of such a diagnosis. Arthur wasn?t a young man even back in the mid-1990s, but his brain still seems to be working pretty well today!

Of course this is hardly a unique tale. There are many other men today who have been living well with advanced forms of prostate cancer for a decade and more. We need to keep building on that realization ? whether we are patients, healthcare professionals, family members of patients, or just educated consumers. Reacting with fear to an initial diagnosis of prostate cancer is still very ?normal? today. Hopefully, by about 2020, maybe we will have managed to demonstrate that such a reaction is no longer sensible because the management options are so much better than they were back in 1995-7 when Arthur first started to offer men his on-line assistance.

27-pound lobster caught in Maine

A massive lobster was nabbed off the coast of Maine earlier this month. It's now being held at the state's aquarium, though it will be released soon.

A giant 27-pound lobster has been caught off the Maine coast, but it won't be going into a pot of boiling water.

Cushing shrimp fisherman Robert Malone caught the monster crustacean in his nets Friday, said Maine State Aquarium Director Aimee Hayden-Rodriques. Malone gave it to the Department of Marine Resources for its aquarium in Boothbay Harbor.

The beast weighed in at 27 pounds and measured nearly 40 inches long. It was dubbed Rocky because it was caught in the Rockland area, but Hayden-Rodriquez said it could have been named for Rocky the boxer because its claws are big enough to pack quite a punch.

The lobster is the biggest ever brought to the aquarium, surpassing the previous biggest one by 4 pounds. It's downright huge compared to lobsters people eat, which typically weigh between 1 and 2 pounds.

The biggest lobster on record was caught off Nova Scotia and weighed more than 40 pounds.

Hayden-Rodriquez says Rocky will soon be released into the ocean because he'll fare better in the wild.

The official Temple Run for Android still isn't out, anything else is just malware

A lot of excited Temple Run fans may have thought the day had finally arrived this morning. Despite having previously confirmed that an announcement would be made via their Facebook page, probably the most convincing fake of all showed up today in the Market. 

The 'developers' were shown as being Imangi Studios, the actual developers of the iOS smash hit. All seemed well, until you actually installed the app. Reports were flooding in suggesting that no levels would be available for another four days. Of course the alarm bells should at this point start ringing and anyone who had downloaded this should uninstall immediately. Imangi -- the real Imangi -- took to Twitter to try and get the word out. Thankfully the malicious app has been removed from the Market. 

To try and calm down the fever pitch that anticipation has reached for Temple Run, Imangi have offered some further information on the release -- you guessed it, on their Facebook page. Everything is a scam, until they say otherwise. They have promised to deliver at least a weeks notice prior to launching in the Market, which rules February out of the equation. The message is clear though folks, you might be excited for the game, but there are some horrible people out there who will stop at nothing to take advantage of that. However, hit the source link below to head over to the official Temple Run Facebook page. After all, that's where everything will be revealed.

Winter storm slams Midwest; NY, New England up next

A powerful winter storm is making a mess from Iowa to the Northeast. The Weather Channel's Mike Seidel reports from Chicago.

Updated at 7:23 a.m. ET: A winter storm?left a?"stripe of snow" across the Midwest before heading toward upstate New York and northern New England on Friday morning, weather.com reported.

Meteorologist Tim Ballisty predicted that Chicago could receive "a half foot or more" of snow.


"On Thursday into Friday, look for a stripe of snow to lay down from the Dakotas into Iowa, northern Illinois, far northern Indiana and lower Michigan," he added. "The bulk of the heaviest snow will fall south of Milwaukee -- closer to the Wisconsin/Illinois border."

Appearing on NBC's TODAY on?Friday, weather.com's Mike Seidel reported that at least 130 flights had been delayed at Chicago's O'Hare International Airport. He added that prior to Thursday,?Chicago had received only 16 inches of snow all winter.

The storm also?dropped several inches of snow over parts of North and South Dakota on Thursday with?some places getting?more than a foot of snow.

"Look for areas north and west of Detroit, such as Flint, to pick up some of the heaviest snow across lower Michigan," Ballisty said. "It's here where we anticipate snowfall amounts to surpass 6 inches.?Still, a decent snowfall will make its way into the Motor City on Thursday night with the heaviest snowfall rates occurring during the overnight hours."

Ballisty predicted that?the storm would march into upstate New York and?northern New England on Friday morning.

"Look for the heaviest snow to fall over the higher terrain of northern New York and Vermont," he said.?"This includes Burlington, Vermont where snow totals could exceed?half a foot."

Boston was also?expected to be hit with a "slushy inch or two," Ballisty added.

From seared meat sealing in juices; to 8 glasses of water a day, there are certain food ?facts? that are so established, we don?t even think about them. Well we?re here to blow your mind (metaphorically, no need to hide) with our guide to food myths that are definitely just myths.


Searing meat

The myth: Searing meat seals in juices.

Why? In 1850 German chemist Justus Von Liebig published the claim that searing meat (putting it in a very hot pan and browning all sides quickly) sealed in the juices, resulting in a juicier (and tastier) dish.

Busted: Compared to boiling of meat, a popular and widely used cooking method at the time, he was right that it was juicer ? but the food science is bunkum. While it sounds like it makes sense (it makes a harder exterior the juices can?t escape from!) meat does not ?work? like that. Searing meat actually causes greater loss of liquid due to the higher temperatures used. What searing meat does do is create a lovely flavour, better texture and colour to the meat. That?s why chefs still do it today.


You must drink 8 glasses of water a day

The myth: To keep healthy you should be drinking at least 8 glasses of water a day.

Why? It takes the body, roughly, two litres (8 glasses) of water to metabolize a diet of 2000 calories. If your body ?uses? 8 glasses of water a day, you should be replacing that water everyday.

Busted: Firstly, the science is true, on average your body does use up 8 glasses of water a day. But the myth you need to drink 8 glasses of water a day isn?t true. Why? Water is in everything; your body doesn?t just get water from just water. Fruit and vegetables are on average 80% water, meat is between 50% and 80% water and even bread is almost 35% water! Quite simply, if your body couldn?t break the food down and extract the water, we wouldn?t be alive today. It?s also been suggested the 8 glasses myth originated from Doctors wanting to promote drinking water over sugary fizzy drinks. It?s certainly true that drinking water is good for you (in the sense that it contributes to nearly all of your bodily functions, staves off hunger and dehydration), but the ?8 glasses? rule is far from a rule.


Eggs contribute to high cholesterol

The myth: Eggs are high in cholesterol and should be avoided by those with high cholesterol and/or on a diet.

Why? Egg yolks do indeed contain a fair bit of cholesterol, the average egg contains 212 milligrams. On par with single servings of liver, shrimp or duck meat.

Busted: Eggs have got a bad rap amongst those with high cholesterol and/or on diets. ?They?re really high in cholesterol!? people proclaim, and avoid them like an egg-y plague. While it is true that the egg yolk contains cholesterol, the Harvard Medical School points out that very little of it makes it into your bloodstream, which is where cholesterol does the damage. Not only that but your body?s cholesterol levels are influenced by saturated and trans fats; eggs contain very little saturated fat and zero trans fats. What eggs do contain is: protein, vitamin A, B, D and E, choline, iron, calcium and potassium! So while we?re not suggesting those trying to watch their weight or avoiding high cholesterol go out and eat a 20 egg omelette, they shouldn?t be avoiding them in fear. Eggs are packed with loads of good stuff and won?t noticeable affect your cholesterol if eaten as part of a balanced diet.


You should rinse pasta after you?ve cooked/drained it

The myth: After your pasta is cooked and drained, rinse it with hot water for a hot dish, cold water for a cold dish.

Why? Erm, because ? people have been told to? This seems to have originated as one of those old housewife tales (old housewives make up a lot of tales apparently) without any real reason why it should be done.

Busted: Cooking pasta releases starch, when you drain it, a thin layer of starch still clings to the pasta. Rinsing the pasta washes off this layer; stop doing that! The starch helps sauces to cling to the pasta, which is exactly what you want to happen. Not only that but pasta is constantly absorbing liquid, keeping a little cup of the starch-y water aside after you drain the pasta is really useful if your pasta & sauce is looking a little dry, just add in some of the starch-y liquid. You only need to rinse your pasta if you?re making a cold pasta dish ? the thin layer of starch is what causes the pasta to stick together when it goes cold.


Fresh fruit is better for you than dry/frozen/canned fruit

The myth: Fresh fruit is much better for you than fruit that has been dried, frozen or canned.

Why? When you do any of those 3 processes to fruit, it takes away many of the nutrients that make fruit good for you.

Busted: This is only true for one thing: vitamin C. Fresh fruit is best for that and has more than dried, frozen or canned fruit. But otherwise dried, frozen and canned fruit contains just as many nutrients as fresh fruit. So next time you?re snacking on a few raisins, don?t let anyone tell you they don?t count towards your five-a-day! Just one tablespoon of dried fruits will count as one ?portion? of your daily intake.

NEWARK, N.J. (AP) ? Jordan Theodore started talking about his last chance to play in an NCAA tournament in the preseason. The senior guard wants desperately to be part of the field of 68.

On Tuesday night, he was nearly perfect in helping Seton Hall get the signature win it needed for its NCAA tournament resume. He had a career-high 29 points on 8-of-11 shooting, including making all five 3-point attempts, in a 73-55 victory over No. 9 Georgetown on Tuesday night.

"We just needed this one so bad. I was jacked up for this game," said Theodore, who also had five assists and four rebounds. "I didn't want to blow another opportunity like we did my sophomore year when we were on the bubble. I have a little experience in being on the bubble. I want to dance so bad. I can taste it we're so close.

The Pirates (19-9, 8-8 Big East), whose best win this season was over Connecticut on Jan. 3, have two games remaining ? Rutgers and DePaul ? to solidify what would be their first NCAA tournament bid since 2006. Their RPI is in the high 30s and it never hurts to be from the Big East, which a record 11 teams in the field last season.

Still, nothing is official until Selection Sunday.

"What could be better than having a game like this with your fate in your hands," said Herb Pope, Seton Hall's other senior. "We stayed solid. We shot terrific today."

Then he started talking about Theodore.

"That's my little brother and we're going out together," Pope said. "We moved one inch closer to our goal. He came out on fire and we rode the wave."

Seton Hall put on quite a shooting performance against one of the conference's best defensive teams. They shot 61.0 percent (25 for 41), the best against the Hoyas this season and only the third time any team shot 50 percent or better. Georgetown (20-6, 10-5) came in allowing 38.4 percent shooting this season.

Seton Hall finished 8 of 13 (61.5 percent) from 3-point range, and that came against a team that was allowing 27.6 percent from beyond the arc this season. The previous best effort from 3-point range against Georgetown was 44.4 percent by Marquette, the only time any team shot better than 40 percent.

"We thought we play pretty good perimeter defense, but we were atrocious tonight," Georgetown coach John Thompson III said. "They hit some shots early and we got frustrated early and that frustration carried over to our offense. That was the crux of everything.

"We didn't find a way to make their shots difficult. They shot the cover off it and we didn't respond."

Theodore, who has talked about making the tournament for the first time since the preseason, and fellow senior Herb Pope dominated the run that gave the Pirates a 58-40 lead with 7:53 to play. They combined to score 10 of the Pirates' 12 points in the run that gave them the 18-point lead.

"I think that's the most complete game a guard has played in this league all year," Seton Hall coach Kevin Willard said of Theodore. "Running the game, controlling the game, shooting. He did it all. We just had to get the ball in his hands because he was going to make shots."

All but three of them.

"Jordan Theodore played one of the better games we've seen all season. He didn't make any mistakes. He totally controlled everything and we didn't have any answers for him," Thompson said. "He hit a lot of open shots, but he took shots that were contested that went in. Whether we played man or zone, when a good player has a game like that, he's hard to stop."

Greg Whittington had nine points to lead the Hoyas, who had won four of five and who would have clinched a first-round bye in the Big East tournament with a win. Georgetown struggled offensively as well, finishing 19 of 47 (40.4 percent) from the field, including 5 of 15 from 3-point range. It was the first time this season the Hoyas failed to have a player reach double figures.

The Hoyas won the last two meetings and six of the last seven against Seton Hall.

Fuquan Edwin and Brandon Mobley both had 10 points for Seton Hall, which led by as many as 22 points late in the game.

"It was a huge win for us," Willard said. "This is one of the best defensive teams in the country and to play the way we did against them is pretty impressive. ... We didn't punch our ticket yet, but we had to win this or not."

YANGON, Myanmar (AP) ? Myanmar election authorities lifted restrictions on political campaigning Monday in an unusually swift response to complaints by pro-democracy leader Aung San Suu Kyi's party.

The National League for Democracy said earlier in the day that the restrictions risked making upcoming by-elections unfair.

The state Union Election Commission's decision to lift all restrictions was unusual. Bureaucratic wheels grind slowly even where there are no political hurdles in the country where an elected, nominally civilian government took office almost a year ago after a half-century of military rule.

NLD spokesman Nyan Win had said the party was facing difficulty in getting permission to use public venues for its meetings ahead of the April 1 polls.

"What we want is fair play but the restrictions have increased lately. It is very difficult to say that the upcoming by-elections could be free and fair," Nyan Win told reporters.

Later Monday, however, he said the state Union Election Commission had informed the party that "all restrictions are lifted for the organizational activities."

"There is now a flicker of hope," Nyan Win commented.

Suu Kyi, a former political prisoner and 1991 Nobel Peace Prize laureate, is running for one of the 48 parliamentary seats being contested in April. Her party overwhelmingly won a 1990 general election but the military refused to allow it to take power.

The NLD boycotted a 2010 general election, saying the rules were unfair. It agreed to rejoin electoral politics last year when the new military-backed elected president, Thein Sein, began implementing democratic reforms.

The government has released political prisoners and amended some election laws among other changes, while arguing that Western political and economic sanctions imposed because of the repression under the past military regime should be lifted.

The U.S. and other nations have specifically cited a fair election as a benchmark by which Thein Sein's administration will be judged.

Nyan Win, also the NLD's campaign manager, had said some government organizations blocked the party's campaign activities even though the election commission had approved them.

He said Sports Minister Tint Hsan had objected to the NLD's use of football fields in three constituencies ? Hlegu in northern Yangon, the central city of Mandalay and the Irrawaddy delta town of Phyapon ? after local election commissions had approved such meetings.

Tint Hsan's son, Phyo Ko Ko Tint Hsan, is a candidate in the by-elections for the ruling military-backed Union Solidarity and Development Party.

Nyan Win also said the election commission in Mogaung township in Kachin State had refused to allow the NLD to organize in a village called Namti on security grounds.

Sporadic but sometimes fierce fighting has been waged in Kachin state between government troops and ethnic Kachin rebels, who have long sought more autonomy and have faced increased repression in the past year.

"The authorities said they will not give permission for the NLD to campaign in Namti village because of security reasons. If security in the region is good enough for the government to hold elections, then it is inappropriate to ban organizational activity based on security grounds," Nyan Win said.

Nyan Win said the party would now be allowed to campaign in Namti. Suu Kyi is scheduled to visit Kachin State this Thursday and Friday on an organizational trip.

Report: Calif. killer's letter cites more bodies

(AP) ? Sacramento television station CBS-13 has received a letter it says is from serial killer Wesley Shermantine claiming he knows of sites where the bodies of additional victims were dumped.

The station reports (http://cbsloc.al/yKaUOu) that in a letter with Shermatine's signature from San Quentin State Prison, the convicted killer says there are two locations where his accomplice, Loren Herzog, disposed of two victims.

Shermantine says he hasn't revealed the locations because he hasn't been paid $33,000 promised by bounty hunter Leonard Padilla.

Padilla, who recognized Shermantine's handwriting in the letter, says he's working to establish a trust account before paying. Padilla says San Joaquin County sheriff's officials have a copy of the letter. They didn't immediately return calls from The Associated Press seeking comment on it.

Shermantine and Herzog, known as the "Speed Freak Killers," are suspected in as many as 20 murders. Herzog committed suicide last month after being told Shermantine was cooperating with authorities.

Authorities have been searching a farm in Linden based on information provided by Shermantine.

NKorea threatens to attack SKorea over drills

(AP) ? North Korea has threatened to attack South Korea accusing it of planning live-fire drills near their disputed sea border.

The North's military said in a statement Sunday that South Korea planned artillery drills from five front-line islands off the western coast on Monday.

It says the drills are "reckless military provocation" and the North will launch "merciless" strikes if the training is implemented.

The North warned people living on the islands to evacuate before the drills start.

South Korea regularly conducts artillery drills from front-line islands but it's not immediately known whether the South plans to conduct such drills on Monday.

A 2010 drill triggered a North Korean artillery bombardment that killed four South Koreans.

(AP) ? Pope Benedict XVI on Saturday brought 22 new Catholic churchmen into the elite club of cardinals who will elect his successor, in a greatly simplified ceremony that took account of evidence the 84-year-old pontiff is slowing down.

Benedict presided over a ceremony in St. Peter's Basilica to formally create the 22 cardinals, who include the archbishops of New York, Prague, Hong Kong and Toronto as well as the heads of several Vatican offices.

Preparations for the ceremony have been clouded by embarrassing leaks of internal documents alleging financial mismanagement in Vatican affairs, and reports in the Italian media of political jockeying among church officials who, sensing an increasingly weak pontiff, are already preparing for a conclave.

None of that was on display Saturday, however, amid the pomp of the consistory that brought to 125 the number of cardinals under age 80 who are thus eligible to vote in a papal election.

That said, each of the new cardinals did make a solemn pledge to keep church secrets upon accepting their new title, ring and three-pointed red hat, or biretta, from the pope.

Reciting the cardinals' traditional oath of loyalty, each one pledged to remain faithful to the church and to "not to make known to anyone matters entrusted to me in confidence, the disclosure of which could bring damage or dishonor to Holy Church."

Benedict was wheeled into St. Peter's Basilica aboard the moving platform he has been using for several months to spare him the long walk down the center aisle. Benedict, who turns 85 in April, spoke in a strong voice as he told the cardinals they will be called upon to advise him on the problems facing the church.

In remarks at the start of the service, Benedict recalled that the red color of the three-pointed hat, or biretta, and the scarlet cassock that cardinals wear symbolizes the blood that cardinals must be willing to shed to remain faithful to the church.

"The new cardinals are entrusted with the service of love: love for God, love for his church, an absolute and unconditional love for his brothers and sisters even unto shedding their blood, if necessary," Benedict said.

Benedict has been slowing down recently. His upcoming trip to Mexico and Cuba, for example, is very light on public appearances, with no political speeches or meetings with civil society planned as has been the norm to date. Even Saturday's consistory was greatly trimmed back to a slimmer version of the service used in 1969: only one of the cardinals actually read his oath of loyalty aloud, while the others read it silently to themselves simultaneously. A reading was cut out, as was a responsorial psalm.

At the end of his remarks, Benedict said: "And pray for me, that I may continually offer to the people of God the witness of sound doctrine and guide the holy church with a firm and humble hand."

Of the 22 new cardinals, seven are Italian, adding to the eight voting-age Italian cardinals named at the last consistory in November 2010. As of Saturday, Italy will have 30 cardinals out of the 125 under age 80.

That boosts Italy's chances of taking back the papacy for one of its own following decades under a Polish and a German pope, or at least playing the kingmaker role if an Italian papabile, or papal candidate, doesn't emerge.

Only the United States comes close, with 12 cardinals under 80, including New York Archbishop Timothy Dolan and Cardinal-designate Edwin O'Brien, the former archbishop of Baltimore who is now grand master of the Order of the Holy Sepulcher, which raises money for the church in the Holy Land.

The consistory class of 2012 is heavily European, reinforcing Europe's dominance of the College of Cardinals, even though two-thirds of the world's Catholics are in the southern hemisphere. All but three of the new under-80 cardinals come from the West, along with a Brazilian, an Indian and a Chinese.

EDINBURGH, Scotland, Feb. 15 (UPI) -- The Scottish government announced it gave approval to its first commercial wave power array, which could deliver power to more than 1,000 homes.

Scotland announced that two wave energy converters will be installed at an existing facility that operators Aquamarine Power will use to test as part of a wave energy array.

"This array will supply more than 1,000 homes with clean energy and should the demonstrator be successful the technology being developed here will power hundreds of thousands more homes in future," said Scottish Energy Minister Fergus Ewing.

Edinburgh said each converter can generate as much as 800 kilowatts of electricity. Each of the converters serves as a demonstrator, the government said, though an entire array would later become the first to link up to the national electricity grid.

"Scotland has unparalleled natural resources, with a 10th of Europe's wave power, and these new devices will help us to fully harness the huge energy resources around our own coast and contribute to global efforts to expand clean, green electricity generation," said Ewing.

Scotland has some of the most ambitious renewable energy targets in Europe. The government aims to meet 100 percent of the electricity demand through renewable energy resources by 2020.

International Childhood Cancer Day: EU Directives tackled

A European Parliament event to discuss how EU legislation has negatively affected the treatment received by children and adolescents has marked International Childhood Cancer Day - 15th February.

The meeting was hosted in association with the European Society for Paediatric Oncology (SIOPE) to raise awareness of the many hurdles faced by patients and those who care for them as a result of the EU Clinical Trials Directive (CTD).

The Directive (2001/20/EC) has been one of the most controversial pieces of European legislation related to health since its introduction in 2004; seen by many to be having a detrimental effect on academic-led clinical research in Europe. Member of the European Parliament Glenis Willmott, who hosted the event in Brussels, Belgium, stated: "EU countries are still using different standards for clinical trials, which means researchers have to apply multiple times for a clinical trial, with different applications.

This makes the whole process time-consuming, expensive, and sometimes futile. That's why we want to make sure all EU countries are playing by the same rules. We need to do everything we can to encourage more research and more clinical trials, specifically designed for children and adolescents".

Although the objective of the Directive was to standardise the regulation and quality of trials, there has been a lack of coordination and huge increases in bureaucracy and administration to meet EU CTD requirements, especially for non-commercial investigator-led, multinational trials.

The CTD is currently being revised by the European Commission, with a proposed revision expected to be published in September 2012 for legislative review by the European Parliament and Council.

While each major type of childhood cancer is individually rare, 1 in 500 will be affected by cancer during childhood- cancer remains the leading cause of death from disease in children and young adults. Clinical trials in this field have provided the evidence base on which current best practice is built and are complex treatments.

The international paediatric oncology community has successfully raised survival, from less than 20% of young people surviving cancer to about 80% over the last four decades. The principal reasons for this are the creation of international networks that have harmonised treatment protocols across geographical borders and delivered high-quality clinical trials.

Currently, over 80% of chemotherapy agents that form the backbone of treatments for children with cancer and standard practice for over 40 years are used 'off label', due to lack of economic interest from the pharmaceutical industry.

SIOPE President Dr Ruth Ladenstein commented: "While clinical research is a necessary tool to combat the burden of cancer the current bureaucratic workload of trial activation in Europe is much too high. Moreover, we are dependent on the pharmaceutical industry to apply for licensed indications and there currently is no economic interest, nor sufficient incentive for the industry to engage in this field of off- patent drug development, especially given the rarity of childhood cancers."

As the treatment provided to young people with cancer in Europe has been based upon years of experience and success, there is a need for an introduction of proportionate risk management to the EU Clinical Trials Directive. "It is paramount that the European Commission takes a reasonable approach when addressing the associated risk of standard care drugs in off-label use that have proven to be successful in orphan indications like paediatric cancer", Ladenstein pointed out.

In addition, the implementation of the EU CTD in individual Member States was accompanied by a variety of interpretations and diversity rather than harmonisation of the conduct of clinical trials. Some Member States consider all off-label drugs within a trial as IMPs whilst others restrict this definition to one drug only specifically under investigation within a randomised trial.

The current revision of the EU Clinical Trials Directive by the European Commission represents a major opportunity to simplify approval and monitoring requirements, provide greater clarity on the scope of the Directive, including the definition of an IMP and to simplify approval and monitoring requirements for low risk trials delivering standard care within a scientific setting.

The current revision of the EU Clinical Trials Directive by the European Commission represents a major opportunity to simplify approval and monitoring requirements, provide greater clarity on the scope of the Directive, including the definition of an IMP and to simplify approval and monitoring requirements for low risk trials delivering standard care within a scientific setting.
